TUITION ASSISTANCE PROGRAM GUIDELINES
APPLICATION & AGREEMENT
Effective January 1, 2015
Purpose
To provide financial assistance through tuition reimbursement to eligible employees of Company Name who want to further their education.
Eligibility
All regular, full-time employees of Company Name who are in good standing. Employees who are covered by a collective bargaining agreement must first discuss their involvement in the Tuition Assistance Program with their local union representative and ask the union representative to contact the Human Resources representative in their region.
Applicable Courses of Study
Tuition assistance is available to employees pursuing courses related to their work or work related courses leading to an associate’s degree, bachelor’s degree, or a master’s degree from an accredited school. Prior approval of course of study is required (see “Requirements” section).
TUITION ASSISTANCE PROCEDURES
Overview
Tuition assistance is offered in the form of reimbursement up to $x,xxx per calendar year and a lifetime maximum of $xx,xxx. Reimbursement is for tuition (class cost) only. No reimbursement will be made for books, lab fees, etc.
Prior to starting any courses, the employee must complete the “Tuition Assistance Application & Agreement” form (page 3) which commits to one (1) year of continued employment with the company after receiving the reimbursement. Upon receiving the required approvals, the employee will enroll in the selected course(s) at the accredited institution and pay tuition costs at that time.
After successful completion of the course with a grade of “C” or better or a “Pass” in a “Pass/Fail” enrollment, the employee must submit the tuition receipts and transcript for the class, along with a copy of the approved “Tuition Assistance Application & Agreement” form, to Corporate Human Resources in order to receive tuition reimbursement. All forms must be sent to Corporate Human Resources for review and processing. Reimbursement checks will be sent to the employee’s home address.
Steps to Follow
- Complete the “Tuition Assistance Application & Agreement” form (page 3) and obtain the required approvals prior to the start of the cours
- Forward the completed application to coordinator name at xxxxxxx@Company Name .com in order to obtain final approval from the director of organizational development in the Corporate Human Resources Departmen
- Company Name ’s Corporate Human Resources Department will review and process your applicatio Once this is completed, Human Resources will return a copy of the approved application form to you. Be sure to keep this copy.
- Once you have successfully completed the course(s), submit a copy of your approved application, along with your original payment receipts for the course(s) and a transcript from the school.
- Your reimbursement check will be processed and mailed to the address that is on file with Corporate Human Resour
Requirements
In order to qualify for tuition reimbursement, courses must be approved in advance by your immediate supervisor, your area’s assistant vice president (AVP)/vice president (VP) or above and by the director of organizational development in Corporate Human Resources. The degree program and/or courses must be in a field of study that is beneficial to the company by maintaining or improving your skills in the performance of your current or potential future job with Company Name .
Courses cannot be attended if attendance would interfere with the employee’s ability to perform his/her job. If Company Name management, in its sole discretion, determines that job performance has been caused to suffer, approval of further course assistance may be denied.
Notice to Eligible Employees
Employee must maintain satisfactory job performance while attending classes. If the employee fails to successfully complete the enrolled class with a grade of “C” or better or a “Pass” in a “Pass/Fail” enrollment or cannot maintain satisfactory work performance, the employee will not be reimbursed for that enrolled class. If employee voluntarily leaves the company or is terminated for cause prior to one year from the date of each reimbursement check, the employee becomes responsible for tuition costs and repayment to Company Name of any reimbursement checks issued within the one year timeframe. Such repayment shall be deducted from employee’s paychecks issued upon termination.
Company Name reserves the right to modify or suspend this Tuition Assistance Program at any time, or to limit the number of participants enrolled in it at any one point in time. If changes are made to the Tuition Assistance Program, reimbursements already approved will be honored under the terms existing when an application was originally approved.
The taxability of payments under this Tuition Reimbursement Program will be in accordance with IRS guidelines in effect when disbursements (reimbursement of tuition) are made.

This Tuition Assistance Agreement is made and entered into by and between [print your name] (You) and Company Name. In consideration of your entering into this Agreement and Company Name ’s payment of your tuition expenses as identified below, Company Name and You agree as follows:
- Company Name will assist in your tuition expenses upon satisfactory completion of agreed-upon course work in accordance with Company Name ’s Tuition Assistance Program Guidelines, Application & Agreement.

You agree that Company Name may deduct the amount due from any paychecks issued upon termination. Any additional costs will be subject to payment directly from You within 30 days from the last date worked. You further agree that You are liable for all reasonable collection costs, including attorneys’ fees necessary for the collection of any amount not paid when due.

- All paperwork, transcripts, and/or grades received after December 21st of any given year will show as reimbursement for the following calendar year.

- Company Name will reimburse you for any applicable tuition fees for currently enrolled courses in the event a new work assignment prevents you from completing the currently enrolled courses.
